Prior to your first appointment, we ask that all clients complete initial intake paperwork through our patient portal at cmcounselintouch.insynchcs.com. All new clients are automatically registered with a portal account where you can access forms, upcoming appointments, and account balances. If you have difficulty accessing or setting up your account, please contact us at info@cmcounsel.com. In the event you cannot access the portal, we have provided downloadable PDFs (below) that you fill out and drop at the office, return by fax, or e-mail to info@cmcounsel.com. By completing your paperwork ahead of time, you will spend less time filling out forms at your first appointment and more time with your clinician.
